	工作簽証申請表格
	身分証影本
	二吋照片一張
	護照
	簽証費
	僱主出具的擔保信函（保證內容包括表演節目或影片拍攝目的、工作人員職稱、工作期限、

　　   所有人員之住宿、往返機票費用。）

紐西蘭演藝公會出具之信函 外國演藝人員，經記人，宣傳公司，或其工作人員在前往紐西蘭從事任何表演活動前， 須先向紐西蘭相關演藝公會報備，如該公會對其表演或影片拍攝無任何異議，將出具同意信函。
